When they first started to experiment on it they used white painted tractor tyres before they finalised the layout; the local youths had great fun re-arranging them overnight.

Take it from me, it's even more scary to meet it head on so to speak. I think there is at least one more similar. Smaller ones with only 2 islands are fairly common though.
The main rule is give way to traffic from the left.
